Assistant Director of Memory Care

Position Summary: This position is responsible for assisting the Memory care Director in promoting and integrating the mission and philosophy of Senior Lifestyle Corporation. The Assistant Memory Care Director in conjunction with the Memory Care Director is responsible for overseeing compliance with all community policies and procedures and applicable state regulations as they apply to Memory Care. The Assistant Memory Care Director will implement and oversee the Memory Care points of distinction established in the areas of Embrace Wellness, Embrace Enrichment, Embrace Creativity, Embrace Connection, Embrace Challenge and Embrace Teamwork. Essential Functions: • Conduct orientation and ongoing training of Memory Care Team employees in conjunction with the MCD, accordance with employee training policy and regulatory guidelines. • Manage schedule and attendance following the attendance policy, notification to MCD if changes occur • Assist with care assignments on the floor when there is an opening in the shift or as needed • Manage Memory Care employees in partnership with the MCD • Meet weekly with MCD to discuss concerns, schedule, staffing, family issues, resident behaviors • Leads daily huddle when the MCD is absent • Attends overnight huddle 1x/month • Completes management rounds 1x/week with MCD • Promote the company’s Embrace philosophy in all aspects of his/her role to include interactions with resident, family, and staff • Organize and order Memory Care team supplies • Oversee and take attendance in Sagely when leading programs • Oversee the Adult Day program in conjunction with the Memory care Director. • Oversee the daily care assignment to ensure they are updated and reflect current changes • Lead high quality innovative programs in Memory Care to include a minimum of 3-5 self-lead programs daily. • Implement and monitor programs and processes to promote excellence in programming while maintaining a high level of resident/family/staff satisfaction in a fiscally responsible manner in conjunction with the MCD. • Care for residents with dementia • Adhere to the budget guidelines for the department. • Maintain current knowledge of Alzheimer’s/dementia topics and participates in Memory Care Department training as assigned. Provide education, training, and hands on support to community staff. • Perform other duties as assigned

Resident Program Coordinator/Memory Care Activities

Plan, develop, and implement resident lifestyle programming within the community as well as through outside trips and events. Survey and interview residents to determine their past and present interests and talents, as well as their satisfaction with the community’s programs and activities. Recognize the individuality, diversity, and spirituality of all residents when planning activities and celebrating holiday functions. Develop relationships with residents to encourage their participation in activities that enhance their quality of and appreciation for life. Observe residents’ function and behavior and reports significant changes or concerns to appropriate staff. Utilize outside community resources and independent contractors to enrich the variety of programs made available to residents. Assist with developing and managing the annual department budget.   Assist with managing Resident Program Assistants as needed in the absence of the Director of Resident Programs. 

Director of Resident Programs Assisted Living

Foster a fun work environment centered around celebrating the lives of our residents Plan and lead daily programs within the community Develop opportunities for team members from other departments to create and lead programs  Plan and execute special events for residents in collaboration and partnership with the sales and marketing departments Recognize and honor the individuality, diversity, and spirituality of all residents when planning programs, events and holiday celebrations Develop rapport with residents to encourage participation in community programs and events Acknowledge, understand and promote the SLC wellness model by offering opportunities for residents to experience programming in 7 areas of wellness (physical, social, emotional, spiritual, intellectual, vocational, & environmental)  Develop and distribute a monthly calendar of events and newsletter that meets SLC program and brand standards by the first of each month Organize and lead trips outside the community to enhance the opportunities for residents to remain connected to the greater community  Develop partnerships inside of and outside of the community to enhance program and event offerings Promote the lifestyle at the community through an active social media presence and positive partnership with the sales and marketing departments  Oversee the resident ambassador program Support and participate in setting up the seasonal/holiday décor of the community Responsible for keeping program flyers, bulletin boards and digital signs up to date and meeting SLC and Brand standards.   Perform work tasks in a way that preserve each resident’s dignity, privacy, and confidentiality Responsible for hiring, training, developing and mentoring dept employees in accordance with Senior Lifestyle Corporation Human Resources guidelines Secure and maintain all department equipment and supplies Manage the department budget and expenses Document and maintain all records as required to effectively manage the department 

Resident Program Assistant- Part Time

Assist in planning and coordinating all resident activities.  Encourage residents to participate in programs and activities and provide transportation to them as needed. Lead various activities as directed by the Resident Programs Director. Order and maintain supplies for activities as needed.

Licensed Practical Nurse (LPN) – Memory Care/Assisted Living PRN

Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines. May be responsible for the admissions of new residents to include all admission assessments and documentation. Responsible for completing acute and long-term care plans or service plans. May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director. Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Provide educational meetings for residents and staff. Understand roll in the Safety & Disaster Plan. Maintain confidentiality of residents’ and prospective residents’ information.     May be responsible for discharges, assessments, and clinical documentation as needed. 

Caregiver/CNA Assisted Living/Memory Care 2pm-10pm Full Time

Provide kind, caring, and consistent companionship and support to residents. Provide assistance, encouragement, and restorative interventions, as needed, for Memory Care residents to be as independent as possible in their activities of daily living including but not limited to: bathing, dressing, grooming, toileting, feeding, and mobility. Participate in and lead one on one and group activities. Serve residents during mealtime, assist with memory care laundry, and basic housekeeping within the memory care neighborhood. Ensure that resident/family/employee concerns receive timely response and follow up, thus striving to provide a high level of customer satisfaction. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.
