RN Unit Manager – Skilled Nursing

Provide direct and individualized nursing care to assigned residents based on established nursing protocol. Observe and assess for: pain/discomfort, oxygen therapy, all drainage, all dressings, and any incontinent elimination which may have occurred. Provide ignition and ongoing systematic assessment of the physical, emotional, social, educational, and functional needs of the resident/family through observation, interview, and physical examination. Interpret assessment data and information to assist in developing a plan of care. Assure medical orders are transcribed and processed accurately and timely and integrate the medical care plan into the provision of nursing care. Assure that medication administration is correct, according to the established nursing policies and procedures. Provide accurate and timely documentation consistent with the resident plan of care. Administer medication per physician orders. Document properly on MOR. Observe for any adverse reaction. Assemble resident charts, resident records, and assure all necessary form are completed in a timely manner. Assure all monthly summaries are completed in a timely manner for assigned residents. Prepare incident/accident reports for residents as needed per policy, notify AL Director, physician, and family post incident per policy and document accordingly. Assist resident in learning self-care activities. Supervise all resident assignments to nursing team members, and assure special instructions are given to team members. Observe and supervise all meals to ensure residents receive the proper diet and any assistance required. Monitor and organize all material resources necessary for meeting needs of assigned unit. Recognize emergencies and life-threatening situations and initiate appropriate action. Ensure that resident call lights are promptly answered, and their needs are addressed timely and appropriately. Assist with the orientation of new staff members. Ensure that all scheduled activities take place on assigned unit by utilizing activity personnel and/or nursing personnel.

Registered Nurse – Evening Supervisor

Responsible for medication supervision/administration in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines. May be responsible for the admissions of new residents to include all admission assessments and documentation. Responsible for completing acute and long-term care plans or service plans.  May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the Director of Nursing. May be responsible for discharges, assessments, and clinical documentation as indicated.  Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Maintain confidentiality of residents’ and prospective residents’ information. Understand role in the Safety & Disaster Plan. Provide educational meetings for residents and staff.

Med Tech – Skilled Nursing

Responsible for supervision of self-administered medication for the Skilled Nursing Department in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines as directed by the LPN/LVN/RN and/or the Manager Nurse and/or the Director of Nursing. Count and sign on approved form, all narcotics with approved personnel before accepting medication keys.  Contact a supervisor immediately with discrepancies. Order all necessary medication and treatments from appropriate pharmacy or notify responsible party when supply is no less than 7 days. Notify supervisor when supply is less than 2 days and medicine hasn’t been received. Responsible for receiving and verifying all medicine received is accurate according to pharmacy label and MAR.  Notifies pharmacy and DH&W of any discrepancies. Responsible for proper storage of all medications and ensuring all lotions, ointments and liquids stored separately from oral medications. Responsible for dating all appropriate vials or bottles at time of initial opening and following pharmacy protocol on length of time they can be used. Responsible for weekly inspections of all medications to ensure all expired medications are removed. Responsible for verifying all current MARs are compared to new MARs at change over time and notifying the supervisor of any discrepancies. Responsible for removing all discontinued medication from med cart and giving it to the supervisor. Provide personal care to residents to include, but not limited to, bathing, skin care, oral hygiene, grooming, dressing, incontinence care, and eating, etc. Provide general nursing care to include, but not limited to, vital signs, temperatures, blood pressure, and weights. Document appropriate information in the daily log as required and notify supervisor of all concerns. Answer calls and assist residents with needs as assigned. Responsible for daily cleaning of all medicine carts at end of shift and restocking as needed.  Responsible for weekly deep cleaning of all med carts. Attend and participate in staff training and in-services. Must maintain accurate records to include preparation of all medical charts for new admissions and readmissions and ensures medical record is complete within forty-eight (48) hours as Policy and Procedure (P&P) and regulatory guidelines.

$2500 Sign On Bonus – Registered Nurse (RN) -Night Shift 11p-7a

Community Relations Liaison

Respond to all leads received within 30 minutes or less by phone and complete subsequent necessary follow up in accordance with the Lead Nurturing Schedule on the sales microsite. Work with Yardi Team to properly provision CRM access to the appropriate communities. Work with IT to be on the marketing distribution list for assigned communities. Input completed and scheduled activities and notes into the CRM in live time.  Communicate clearly to the community team about scheduled visits and communication with the new lead. Use the Lead Manager function in the CRM to continuously advance pre tour leads to post tour leads.  Send weekly completed activities and highlights/lowlights from the week for each community to the Regional Director of Operations, Regional Director of Sales, and Sales Specialist Send immediate communication to DSM and ED on scheduled tours Understand family needs and refer to the appropriate community if need be. Understand partner offerings, resident requirements, and move in process.  Manage all new, pre-visit leads for the first 30 days in accordance with the Lead Nurturing Schedule on the sales microsite. Consistently meet or exceed visit, mystery shop, and move in goals Develop and maintain a good working relationship with all community-based employees, families, and residents.  Manage the decline and notification of duplicate leads on behalf of the community in accordance with the contractual obligation to decline within 3 business days.  Maintain the CRM daily with completed and scheduled activities, clear and detailed notes, and carry zero past due tasks.  Understand how to run move in and move out reports in Yardi Have excellent working knowledge of events, staff and key employees, resident ambassadors. Travel up to one week per quarter for community knowledge experiences.

Dietary Aide – Full Time or Part Time Openings

Follow and perform duties in adherence to Senior Lifestyle Corporation Dining Policy and Procedures according to the Dining Services Policy and Procedure Manual.   Serve food in accordance with established portion control procedures and ensures that foods follow prescribed modified or therapeutic diets, nutrition/hydration requirements and resident preferences.   Assist or serve meals as necessary and in a timely manner assuring that they are palatable and appetizing in appearance as well as accurate in content to the resident.  This will include resident room service as well as service to designated dining areas.  Setup, deliver, and verify accuracy of meal trays, food carts, and dining areas as instructed.  Upon meal completion team member will remove all soiled items/equipment and deliver to the dishwashing area.  Distribute and collect menus as necessary for each meal period. Assist department in obtaining proper food supplies for the next meal.  Assist cooks in preparation of menu items. Prepare and deliver snacks and other items as instructed and assist in inventory control and stocking. All Dietary Department personnel should follow safety precautions to prevent accidents in food preparation according to our accident prevention in food preparation and prevention of back injury policy. Perform kitchen utility duties such as, but not limited to: sweep and mop floors, buss items from dining areas to dish room, wash and clean small wares and equipment. Clean and sanitize work areas, cutting boards, and table tops as well as removing garbage. Keep work areas, clean, dry and free of hazards. Know and adhere to all company and the local, state, and county health department’s regulations. Ensure that all dining areas are clean and safely operating.   Attend all required training, in services, and staff meetings.  Meet all department training standards through Redi-Learning and/or other necessary training modules as required.

Director of Social Services

Working knowledge and ability to apply professional standard of social work practice in job situations.  Demonstrate satisfactory level of interpersonal skills to interact with residents, families, community, government agencies, and interdisciplinary staff. Able to establish criteria to assure that services provided meet established standards of quality.  Demonstrate assessment skills sufficient to evaluate resident’s behavior, to collect data to evaluate psychosocial needs, risk factors for psychosocial deterioration, and residents’ responses to interventions. Ability to interpret and explain resident behavior to staff in a way that fosters understanding, facilitates treatment, and respect for residents rights. Minimize the use of chemical and physical restraints.  Ability to implement social services interventions that achieve treatment goals, address resident needs, link social support, physical care, and physical environment to enhance quality of life.  Promote and protect resident rights, assist residents to make informed decisions, treat residents with dignity and respect. Protect residents’ personal belongings, report suspected abuse or neglect, avoid the need for physical restraints in accordance with the current professional standards. Support independent expression, choice, and decision-making consistent with applicable law and regulation.  Complete progress notes and track/complete PASRRS.

Social Worker

As part of the admission process, meet with the resident and family to review policies on advanced directives, resident rights, and discrimination. Conduct and document a social services evaluation, including identification of Resident problems/needs. Provide/arrange for social work services as indicate by resident/family needs. Assist residents, as needed, with financial planning. Keep the resident’s family informed of resident problems, personal needs, transfers, and changes of level assignment. Maintain a current list of community resources to facilitate referrals. Working knowledge and ability to apply professional standards of social work practice in job situations.
