Nursing Home Administrator

Act as the on-site executive for all operations, including being the main point of contact for all staff, residents, prospects, community organizations, government agencies and the public.  Maintain budget accountability and cash flow; aggressively anticipate and minimize negative budget variances and deficits. Executive Director

**Multi-Level Administrator License Needed** Act as the on-site executive for all operations, including being the main point of contact for all staff, residents, prospects, community organizations, government agencies and the public when needed.  Maintains resident retention by working with residents and their families, dealing with any issues that arise. Responds and follows up with all walk-ins, phone-ins, mail-ins timely and appropriately. Keeps up to date with information about competitors. Meets all expectations of meaningful contacts, leases and occupancy. Maintain budget accountability and cash flow; aggressively anticipate and minimize negative budget variances and deficits. Hire, train, discipline, and terminate employees in accordance with Senior Lifestyle Corporate policy.  Maintain all local, state, and federal licenses for the community. Housekeeper

  Full Time Tuesday through Saturday 7 to 330 General cleaning of resident apartments, hallways and other common areas. Assist in preparing empty apartments for resident move-ins. Ensure resident/guest satisfaction by responding quickly to needs and requests.  You may be asked to help respond to resident emergencies. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

Registered Nurse – Skilled Nursing

Responsible for medication supervision/administration in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines. May be responsible for the admissions of new residents to include all admission assessments and documentation. Responsible for completing acute and long-term care plans or service plans.  May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the Director of Nursing. May be responsible for discharges, assessments, and clinical documentation as indicated.  Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Maintain confidentiality of residents’ and prospective residents’ information. Understand role in the Safety & Disaster Plan. Provide educational meetings for residents and staff.

Licensed Practical Nurse (LPN) – Skilled Nursing – Evening Shift

Strong evening shift differentials. Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines. May be responsible for the admissions of new residents to include all admission assessments and documentation. Responsible for completing acute and long-term care plans or service plans. May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director. Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Provide educational meetings for residents and staff. Understand roll in the Safety & Disaster Plan. Maintain confidentiality of residents’ and prospective residents’ information.     May be responsible for discharges, assessments, and clinical documentation as needed. 

Assistant Director of Nursing

Responsible for the proper maintenance of medical records to include monitoring documentation in Point Click Care.  Monitor current health records, follow-up on incidents, issues with skilled residents and/or families, and meetings as needed. Develop and maintain a working relationship with professional providers of care to skilled residents.  Assist the direct care nursing staff with admissions, transfers, and the discharge of residents. Monitor and follow-up with falls, infection control, skin issues, unusual occurrences, medication delivery error and any other issues as indicated by the DON.  Maintenance of quality assurance parameters including chart audits, log audits and the scheduling of in-services as indicated. Oversee the daily completion and adherence to documentation as required in PCC. Complete audits as per direction of the DON. Assist the direct care staff with admissions, transfers & discharges. Provide direct care nursing and supervisor responsibilities as indicted. Provide staff in-service and education as indicated. Communicate, clarify and obtain orders, and updates on resident condition, etc. with other Health Care Providers.

Social Worker

As part of the admission process, meet with the resident and family to review policies on advanced directives, resident rights, and discrimination. Conduct and document a social services evaluation, including identification of Resident problems/needs. Provide/arrange for social work services as indicate by resident/family needs. Assist residents, as needed, with financial planning. Keep the resident’s family informed of resident problems, personal needs, transfers, and changes of level assignment. Maintain a current list of community resources to facilitate referrals. Working knowledge and ability to apply professional standards of social work practice in job situations. Able to establish criteria to assure that services provided meet established standards of quality. Demonstrate satisfactory level of interpersonal skills to interact with residents, families, community and interdisciplinary staff. Demonstrate assessment skills sufficient to evaluate residents’ behavior, to collect data to evaluate psycho- social needs, risk factors for psycho- social deterioration and residents’ responses to interventions. Able to interpret and explain resident behavior to staff in way that fosters understanding, facilitates treatment and respect for resident rights and minimizes use of chemical and physical restraints. Implement social service interventions that achieve treatment goals, address resident needs, link social supports, physical care and physical environment to enhance quality of life.
