Director of Health and Wellness

Directly oversee the delivery of care to Assisted Living residents to ensure that emotional, physical, psychological and safety needs are met through all program services and activities. Coordinate, along with the Executive Director, family/resident care conferences and serve as a direct liaison between residents, families, and staff.  Work with residents, families, and other staff to ensure that residents are at appropriate levels of care.  Manage labor costs, supplies, and other line items in clinical budget.  Monitor documentation systems to ensure that all regulatory timeframes for review of assessments and service plans are met and that resident needs are met. Write and review residents’ assessments/evaluations and care-plans with appropriate staff. Assess/evaluate potential residents for Assisted Living level of care.  This may involve assessing/evaluating potential residents in their home or another healthcare setting; must be able to drive to location and perform assessment/evaluation professionally and timely if necessary. Schedule and conduct new resident orientation for all Assisted Living Residents.  Provide, plan, schedule, and monitor staff training programs and in-services for the Assisted Living staff including statutory or regulatory required training. Attend all required training, in-service, and staff meetings. Manage and coordinate the work of all nursing and other assisted living staff, communicating pertinent resident health and safety information.  This includes overseeing and/or completing staff schedules monthly and daily assignments.  Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.  Ensure compliance with all state regulatory requirements including mandatory reporting to State. Ensure that all concern and incident reports are completed, filed and that follow-up is conducted appropriately.  Assist and/or perform in mock surveys.  Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers while adhering to all policies, standards and procedures of Company. Require 24/7 on-call clinical support. Perform other duties as assigned.

Assisted living Attendant- Full Time (All Shifts)

Provide kind, caring, and consistent companionship and support to residents. Provide assistance, encouragement, and restorative interventions, as needed, for Memory Care residents to be as independent as possible in their activities of daily living including but not limited to: bathing, dressing, grooming, toileting, feeding, and mobility. Participate in and lead one on one and group activities. Serve residents during mealtime, assist with memory care laundry, and basic housekeeping within the memory care neighborhood. Ensure that resident/family/employee concerns receive timely response and follow up, thus striving to provide a high level of customer satisfaction. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

Business Office Manager

Maintain basic knowledge of computer software and internet applications. Maintain resident, business office and employee files. Perform Human Resources function by conducting new hire orientation and administering benefits. Assist in answering resident billing issues. Also maintain the ancillary charge worksheet on a weekly basis. Ensure that move-in deposits and rents are deposited in a timely manner. Send monthly delinquent notices and follows collection guidelines. Process resident lease renewal letters. Ensure that payroll is accurately prepared and reported to the corporate office at the time designated. Ensure that accounts payable procedures are processed timely & accurately. Ensure that all proprietary, financial, and resident information is kept confidential. Monitor and record petty cash disbursements and reconcile to replenish.  Process/file workers compensation claims. Respect and maintain confidentiality of the office, it’s records, and restricted information. Understands the safety and disaster plan. Participate in the Manager on Duty program. Attend various community events. Attend all required training, in-service, and staff meetings. Assisted Living Attendant/Caregiver-Full -Time (3-11)

Provide assistance as needed with medication reminders (as applicable), bathing, grooming, dressing, escort service, and other activities of daily living. Provide emotional and social support to residents. Document daily log of assistance. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality. Ensure that resident/family/employee concerns receive timely response and follow up, thus striving to provide a high level of customer satisfaction. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

Certified Medication Technician/ CMA- Full Time (3pm-11pm)

Responsible for the safe administration of medications ensuring the 6 rights of medication administration are followed. Count and document all narcotics that are administered to residents.  Responsible for the total inventory of medications at the community; including but not limited to ordering medications, receiving and verifying medications, storing and dating medications, removing expired or discontinued medications, and notifying the proper parties when medication supply is low. Notice discrepancies regarding medication inventory and report to the appropriate parties.  Responsible for daily cleaning of all medicine carts at end of shift and restocking as needed. Responsible for weekly deep cleaning of all med carts. Responsible for verifying all current MARs are compared to new MARs at change over time. Provide personal care to residents to residents as needed, including bathing, skin care, oral hygiene, grooming, dressing, incontinence care, and eating. Provide general nursing care to include, but not limited to, vital signs, temperatures, blood pressure, and weights. Document appropriate information in the daily log as required.

Memory Care Attendant- Part Time / PRN

Memory Care Attendant/Caregiver Overnight Part-Time (11pm-7am)

Promote the company’s Embrace philosophy in all aspects of his/her role for residents, families and staff. Implement and oversee the six unique pillars of Embrace. Daily oversight of the operations in the Memory Care area in partnership with the Memory Care Director and Health and Wellness team.  Lead high-quality innovative programs in Memory Care. This is the positions primary focus. Person will personally lead a minimum of 5-7 programs per day. Assist in planning, coordinating, and evaluating resident activities. Encourage resident participation in programs. Provide education, training, and hands on support to community staff in partnership with the community managers. Implement and monitor programs and process to promote excellence in programming while maintaining a high level of resident/family/staff satisfaction in a fiscally responsible manner.
